Seminar Sign-Up Instructions:
To register, download the Student Registration form, complete and sign the form, then fax to us at 407.349.0287. You can complete your payment information with our convenient PayPal payment option or mail your check and completed form to our Post Office Box.
For the On-the-water session, in addition to the Student Registation form, download the Participation Agreement, and the Medical Information form, complete and fax to 407.349.0287 or email to isail@usailflorida.com.
